After his residency in Internal Medicine in South Africa, he completed fellowships in Nephrology at the Michael Reese Hospital and Medical Center in Chicago, Illinois, and at the University of Cincinnati Medical Center. He received appointments as Assistant Professor of Medicine in 1974, Associate Professor in 1978, and Professor in 1985. To date, Dr. First has published 292 papers and has written 29 book chapters in the field of Transplantation.

Dr. First has given numerous presentations in the United States and throughout the world. He is a past President of the American Society of Transplant Physicians, and he has served on many committees of the United Network for transplantation in the United States. He is a past chairman of the UNOS Organ Donations Committee. Dr. First was recently elected to the Executive Council of the Transplantation Society. He is a reviewer for twelve different medical journals and is on the editorial board of eight medical journals. He has served as a reviewer for the National Institute of Allergy and Infectious Diseases (NIAID) Review Committee on Transplantation Immunology, and is currently a member of the National Institutes of Health (NIH) Data Safety and Monitoring Board and the Food and Drug Administration (FDA) Advisory Panel on immunosuppressive drugs. Dr. First is board certified in Internal Medicine and Nephrology.
